**Responsibilities**: Nurse Care Planner - Remote / Work-from-Home Role Right at Home Canada In-Home Health Care is one of the largest providers of exceptional home care services to older and differently-abled adults, globally. Right at Home Canada is currently looking for a Nurse Care Planner for British Columbia In this unique work-from-home role, you utilize your excellent communication and relationship-building skills to engage new client inquiries from the region, conduct client care consultations via phone/web-based platforms, and work in tandem with local Care Team Leads to complete in-home client assessments and fulsome, individualized client care plans. Your Care Plan is implemented at the local level by your local Right at Home Care Teams (HCAs, LPNs and companion care staff) with care management conducted by you. You must love working with families and helping them understand the home care and support services that will assist in supporting their loved one in staying home safely and independently. This is a nurse administration role with no client in-person assessments or evaluations. All is conducted through telephonic and virtual means. You must be comfortable with computers and technology and not be adverse to entering client information, care plans and other administration. You are the most responsible person to our client families in all aspects of a care plan design, care service delivery, and the overall client family experience. In addition, you are leading, retaining and motivating your local teams of PSWs who support your client families in various pockets of the province. Our ideal Care Planner: - Licensed to practice nursing in the province of BC (additional provincial licensure an asset) - Has appropriate professional space for a home office - A positive, confident, warm, and approachable person who builds immediate trust and rapport - Ideally you have extensive home care/care planning experience - Experience with virtual care / virtual client assessment and communication an asset - Knowledge of community resources and roles of healthcare professionals (“navigation”) - Knowledge of healthcare organizations and models used in community care - Excellent communication and relationship-development skills - A caring, compassionate person committed to enhancing the lives of those we serve - A resourceful, even-keeled, multi-tasking individual who loves a team environment - Someone passionate about quality care and having a meaningful impact on a family's life - Have a "nothing is too big or too small to ask" attitude - Able to lead teams and build respect - Organized, detailed with excellent client service skills - A self-motivated professional - Fluent in both English and French an asset In this role you will: - Work closely with Right at Home client families, care coordinators, and caregivers on a daily basis - Spearhead the development and delivery of our client's individualized care plans in tandem with your local teams. - Work closely with client's medical professionals to ensure smooth delivery of support - Participate in the initial screening, hiring and supervision of caregiving staff and nurses - Engage directly with the Right at Home management team, business development representatives, and other staff as an integral part of the company’s goal to continue to be Canada’s leader in the delivery of family-funded home health care services **Salary**: $45,000.00-$60,000.00 per year **Benefits**: - Extended health care Schedule: - Monday to Friday - Weekend availability Benefits
