Responsibilities Perform thorough evaluations of daily living and work-related skills, assessing the needs of the person receiving services, and set measurable objectives in conjunction with the rehabilitation team. Adjust treatment as needed to achieve maximum results. Design community reintegration activities to assist the person served in physical and cognitive reconditioning, coordinating with the interdisciplinary team. Help the person understand their needs and limitations and how to manage them during function. Recommend and/or design adaptive equipment to improve function in home, community, and work environments, incorporating adaptive equipment into the rehabilitation program. Develop retraining programs and train staff and family members in activities such as dressing, feeding, grooming, hygiene, and community re-entry skills. Provide training to the person receiving services and others involved in their rehabilitation, analyzing and documenting the results of any training. Apply professional knowledge and skills according to accepted standards of clinical practice to facilitate problem resolution and achieve individualized goals. Report progress to the rehabilitation staff, person served, and family members. Observe, document, and report changes in the condition of the person served to the Clinical Coordinator/designee, attending physician, and other team members. Provide treatment progress and status documentation in accordance with the Plan of Treatment. Participate in team processes, provide input, and develop effective treatment strategies. Attend case conferences and family educational meetings. Develop, teach, and assess the plan of treatment. Guide the Rehabilitation Specialist/Certified Nursing Assistant in fulfilling the plan. Use appropriate methods and tools in all teaching and training activities, including discharge preparation. Perform other duties as assigned based on person served, office, or program needs. Qualifications Bachelor's degree in Occupational Therapy Maintain professional license and/or certification as required by the state of practice 1 year of experience preferred Knowledge of rehabilitation techniques related to complex neurological injuries Effective communication skills Current CPR certification Successfully complete any required pre-employment evaluations Good organizational and communication skills with the ability to work independently Reliable transportation Physical ability to perform duties, including lifting, standing, bending, and walking, with or without reasonable accommodations Complete health examination prior to assignment and periodically thereafter as required.
